To display the Hawaiian language properly you need a "macron" or overbar over a character and an `okina or opening single quote. As you can see I can do the `okina (it should really be closer to the character it preceeds) but how can I do the "macron"?

I'm no expert on Hawaiian, but I occasionally use macrons over vowels. Here's how I do it in Nisus Writer Express.
Click the Edit menu (or on some systems the flag menulet representing the selected language), then Special Characters. The OS X Character Palette will open.
Next to View, select either Roman or Unicode.
If Roman, click the "by Category" tab. If Unicode, click "Unicode Blocks" tab.
In the pane on the left, click Accented Latin if you are viewing Roman by category. Click Latin Extended-A if you are viewing Unicode blocks.
Scroll through the matrix on the right until you find the letter-macron combination you want. I think all the vowels are available, but I don't thing any consonants are.
If you use a character semi-frequently, select it and click the Add to Favorites button for simpler access.
To insert a character either double click it or click it and then click the Insert button.
This is cumbersome if you use the characters very frequently. Is there a Hawaiian-language keyboard layout? Failing that, perhaps someone would know how to construct a macro that would insert the characters you want.

Some help, but yes, very cumbersome. Apple shows a Hawaiian language option (unicode?) but I have no clue how to use it with Nisus Express.

I don't really know either, David. You might try switching to Hawaiian and opening the OS X Keyboard Viewer. (If you have more than one language checked as available, the Keyboard Viewer should be available from the menu that appears when you click the flag in the menu bar.)
I turned on Hawaiian, switched to that language and opened the Keyboard Viewer. When I hold down the option key (or the option and shift keys), the Keyboard Viewer reveals some characters that look like what you were describing: vowels with macrons.
Give it a try and see if it does what you need.

Here's how you do it.
Go to the "flag menu". Drag down and select International. Check the box for Hawaiian. This will add Hawaiian to the 'Flag menu".
Then select the Hawaiian flag. Type away, and when you want your macron, type option-u and then the letter over which you want the macron to appear. It is the same procedure used to get the umlaut in a regular USA keyboard, but with Hawaiian it gives you the macron.
It should be very easy and convenient for you.
