I have a fairly long 2 column doc with a high density of internal links.
Sometimes this doc opens with a blank left hand column on the first page (only) and sometimes it opens correctly filling both columns. When the column is blank I cannot force it to adjust, but after a while or if I close and reopen sometimes it does.

Any ideas?
