The following version allows for the possibility that there is no ellipsis. There is a single space after "(" and no space before t "(" or after ")"

To make a link to a file via Insert Hyperlink, drag the file from the finder into the "Link Destination" box, and prepend "file:/" to the text that is created. Clicking the link will reveal the file in the finder.

Use the Hyperlink menu item, located at the bottom of the Insert Menu.

The solution to both problems turns out to be easy: put the cursor in the _header_ at the start of each section before doing page setup.

The agency that issued the form is using Custom Properties to process it. I don't know much about these, but it's likely that only a completed version of the original doc file will be acceptable.
