A small change can be made by changing the alignment of the divider line from bottom to center or top in the style. You could also create a larger gutter and then change the alignment to create more space if the alignment alone is not sufficient.

That's odd. When I increase the padding, it alters the distance between the space between the box line and the first line of text. I believe if you increase the vertical padding on the text box itself, that will give you the spacing you're looking for.

I wrote my dissertation in Palatino and liked it's readability. When it was published it was done in Garamond, which also turned out very nicely. I know that many journals use Century Schoolbook.

Have you tried rebuilding the TOC? If the doc had been created with Roman page numbering and the TOC was built, and then later the page numbers were changed but the TOC was not rebuilt it could result in the mismatch you have. Nisus will do different odd and even page headers if the facing pages option is enabled. To use this check the facing pages option in the sections palette.

You can insert the PDF as a watermark in the background. Tools > Watermark. Select the Watermark from an image file and then select the PDF. You can set the alignment in the dialogue box along with the level of transparency.
